Support Coordination
What is Support Coordination?
Support Coordination focuses on relieving the stress of organising your NDIS supports and aims to build you or your support network’s capacity to understand and navigate the NDIS. Your Support Coordinator provides impartial and independent advice and connects you to mainstream, funded and government disability support services. Having access to professional support is vital to ensure you receive the funding to meet yours and or your loved ones needs and goals.
Specialist Support Coordination may be funded when the NDIS has assessed that a participant has complex and or high support needs. Your specialist support coordination will be provided by a qualified and experienced practitioner such as an occupational therapist, psychologist or social worker. Optcare has a team of qualified support coordinators who will be able to manage challenges in your support environment in areas such as health, education and justice services.
Here at Optcare, our in house support coordinators take a holistic and person centred approach so that we are able to assist you to identify services that meet your individual needs and goals.
Here at Optcare, our team will work with you to help you build the skills to understand and implement your NDIS plan. This may include:
- Assess and identify your support needs
- Finding and connecting you to appropriate supports (mainstream, disability specific and allied health)
- Liaising with NDIS providers on your behalf
- Budgeting your NDIS plan and advising on ways to utilise unspent funding
- Providing education surrounding NDIS funding categories and how to use the NDIS portal
- Supporting you to apply for appropriate accommodation (SIL, Social Housing or Private Rental)
- Assisting you to find meaningful open or supported employment
- Assisting you to apply for relevant assistive technology
- Preparation and support for the NDIS Review
